VNACJ Community Health Center is hiring! Advanced Practice Nurse, Primary Care Center. Visiting Nurse Association of Central Jersey Community Health Center (VNACJ CHC), a federally qualified health center in Monmouth County, dedicated to offering accessible, high‑quality healthcare to New Jersey residents. The Advanced Practice Nurse provides comprehensive primary care services to a diverse patient population at our Primary Care Center in Freehold, NJ, with a strong emphasis on pediatric care. This full‑time role involves evaluating, diagnosing, and treating acute and chronic conditions, delivering preventive care and health education, and managing the ongoing health needs of patients of all ages. The APN works collaboratively with the clinical team to ensure high‑quality, patient‑centered care and supports the daily operations of the practice.
PrincipalResponsibilities
- Supports organization’s mission by striving for excellence in all aspects of the job with a focus on positive interpersonal relationships with co‑workers.
- Provides palliative care or hospice face‑to‑face evaluation services to individual and family clients within established productivity guidelines.
- Provides client teaching to individuals and families when appropriate.
- Monitors quality and appropriateness of services provided.
- Identifies new services or programs relevant to agency’s mission and philosophy.
- Adheres to agency infection control and safety policies and guidelines of primary care regulatory agencies.
- Supervises staff in conjunction with the organization’s managerial and leadership practices.
Minimum Qualifications
- Licensure as an Advanced Practice Nurse in NJ.
- Master’s degree in Nursing from an NLN‑accredited program.
- Certified as a Family Nurse Practitioner with prescriptive authority; maintain a CDS and DEA permit.
- 2+ years of primary or ambulatory care nursing experience required.
- Business or management degree or experience preferred with demonstrated leadership skills.
- Valid driver’s license and reliable transportation.
Working indoors. Exposure to blood, body fluids, and communicable diseases. Tasks include sitting, fine‑motor hand movement, standing, walking, and client/customer contact.
Pay & BenefitsIn compliance with New Jersey’s Pay Transparency Act, the total compensation range for this position is USD $120,000 to $170,000. The range reflects the total package, including base salary, overtime, bonuses, add‑on incentives, special clinical compensation, retirement plans, and all other benefits.
