Location: FHN Memorial Hospital – Freeport, IL Employment Type: Full‑Time FHN is seeking a skilled Nurse Practitioner (AG-ACNP or FNP) to join our Hospitalist team.

As an Advanced Practice Provider at FHN Memorial Hospital, you will play a vital role in our inpatient care team, blending advanced clinical expertise with compassionate bedside manner to manage patient care from admission to discharge.
The Opportunity

• Compensation Range: $110,000 - $138,00

• Clinical Autonomy: Manage your own caseload of patients on the medical‑surgical and acute care floors, including performing admissions, daily rounding, and coordinating discharges.
• Collaborative Practice: Work in close partnership with our Board Certified Internal Medicine Hospitalists, who provide support and consultation while respecting your role as a primary provider for your patients
• Impactful Care: Drive high‑quality outcomes in a rural setting by focusing on patient safety, readmission prevention, and effective communication with primary care providers and specialists.

• Master’s Degree or Doctorate (DNP) in Nursing from an accredited institution.
Current Board Certification:
Adult‑Gerontology Acute Care Nurse Practitioner (AG‑ACNP)

Family Nurse Practitioner (FNP) with significant inpatient/hospital experience will be considered.
• Current Illinois Advanced Practice Nurse (APN) license or eligibility to obtain one.
• Current ACLS and BLS certifications required.

• Excellent communication skills and a cooperative spirit for team‑based care.
