PAY RATE: $19.97 - $23.96 Hourly

JOB SUMMARY

Provide supportive services to the community using the Pathway of Hope strengths-based case management approach to individuals and families with a desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Conduct outreach and engagement services in the community to identify and support potential participants of Pathway of Hope.
  • Conduct client intake interviews to determine eligibility for programs or agency referrals.
  • Provide case management services to clients to include, but not limited to: reviewing intake material for accuracy, assessing client needs, provision of direct assistance, case plan development and compliance, act as liaison to community resources, provide referrals to appropriate agencies, act as an advocate for clients when needed.
  • Maintain an appropriate caseload (at least 10 unduplicated families per fiscal year and carry at least 5-7 active families at any given time). Provide appropriate referrals and services to support individuals and families of the local community.
  • Document and maintain written and electronic case files ethically and confidentially, to include progress, referrals, types of assistance provided, and other information as required.
  • Record, track and submit statistical information as scheduled to maintain compliance with The Salvation Army and grants funders as required.
  • Engage and collaborate by participating in monthly supervision, divisional case manager cluster call-ins, and local Pathway of Hope team meetings driven by the POH Regional Coordinator to ensure best practices.
  • Maintain working relationships with community agencies to provide comprehensive services for participants and to stay informed of current trends and resources available.
  • Collaborate with local leadership to best support clients and their designated goals.
  • In administering programs, uphold the policies, goals and mission statement of The Salvation Army.
  • Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices.
  • Maintain compliance in The Salvation Army's "Safe from Harm" Certification.
  • Assist with answering the phone and doors/guests of the Corps for community needs.
  • Other duties as related to the position and by the managing Corps Officer(s).

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES

Education: Bachelor of Science degree in social work, sociology, criminal justice or another related field. In lieu of a degree, candidates with a minimum of five years' experience in social work or case management with a social service organization will be considered.

Experience: A minimum of three to five years' knowledge and work experience related to Social Services and supporting others through Case Management is preferred. Familiarity with local resources, Continuum of Care and homeless…
