JOB SUMMARY

Friends of Children with Special Needs (FCSN) is a Bay Area non‑profit organization founded in 1996 and focused on helping individuals with special needs and their families find love, hope, respect, and support through integrated community involvement. The FCSN Supported Living Services (SLS) team is seeking an energetic, patient, detail‑oriented, and reliable Case Manager.

ESSENTIAL FUNCTIONS

Under the supervision of the Program Manager, the Living Services Case Manager provides case management service, schedules staffing, supervises Living Service trainers, enhances quality of service delivery and Person‑Centred Thinking (PCT) implementations, conducts ISP meetings, builds and maintains relationships with stakeholders, acts as the designated emergency leader when the supervisor is not available, and establishes collaborations with community partners.

RESPONSIBILITIES
  • Case Management
    • Provide comprehensive case management services to optimize participant success in living arrangements.
    • Coordinate resources; obtain and maintain appropriate living situations and public benefits.
    • Supervise tailored living skill training; monitor participant wellbeing.
    • Arrange regular and as‑needed health check‑ups; promote personal growth.
    • Manage budgeting and financial status; handle safety and emergencies; offer ongoing support.
    • Oversee living service operations and execution of quality service delivery.
    • Develop and implement solutions to overcome obstacles.
    • Supervise and implement behavioral support plans, positive behavioral prevention strategies, and intervention techniques.
    • Develop comprehensive PCT Individualized Service Plans (ISP); conduct quarterly, semi‑annual, and annual ISP meetings with the interdisciplinary team and supervise implementation of objectives.
    • Arrange and supervise bi‑weekly/monthly staffing schedules for household staff; monitor staff EVV logins and oversee their responsibilities.
    • Develop weekly/monthly schedule for living skill training, activities, and community integrated outings based on participant interests, preferences, skills, and needs.
    • Submit annual budget for assigned consumers to Program Manager at least two months prior to consumer birth month.
    • Conduct thorough assessments for clients’ living skills, health, financial status, strengths, challenges, support and training needs.
    • Supervise the quality/maintenance of consumer records, documentation, and operations to meet RCs requirements.
    • Submit SIR APS report to parents, conservators, RC, and/or APS promptly.
    • Maintain effective communication with circle of support, internal and external stakeholders.
    • Communicate effectively with Program Manager regarding participant status, staff performance, and program quality.
    • Carry rotated 24‑hour emergency phone and provide support and crisis intervention.
    • Participate, inspire, and involve staff, participants, and families in FCSN special events and fundraisers.
  • Staff Supervision
    • Provide initial orientation and shadowing for new hires; review performance and give constructive feedback.
    • Conduct ongoing training, facilitate bi‑weekly/monthly apartment meetings, and provide close supervision to promote staff proficiency.
    • Supervise mentor and evaluate staff performance, PCT implementation, and documentation to ensure quality, accuracy, and compliance.
    • Promote open communication and foster a positive, inclusive workplace that inspires and motivates the team.
    • Be a role model; proactively analyse morale and plan team‑building activities to foster teamwork and retention.
    • Conduct semi‑annual and annual performance evaluations.
  • Emergency Preparedness and Handling
    • Document outcomes of fire, earthquake, medical, and community safety drills.
    • Handle emergencies and act as interim leader when Program Manager is not available.
  • Community Representation
    • Represent FCSN in communication with parents, consumers, Regional Center representatives, and other collaborative agencies.
    • Engage in outreach special events and fairs to promote FCSN publicity.
