Registered Nurse - Center

Overview

On Lok PACE is a non‑profit program dedicated to providing comprehensive senior care services in the community. The Center Nurse works under the Program/Center Manager and the clinical supervision of the Nursing Program Specialist, delivering nursing services across the Day Health Center.

• Assess participant needs and develop appropriate nursing care plans upon initial intake and routine reevaluation.
• Implement, direct, supervise, and evaluate nursing care for participants at the Day Health Center.
• Teach, supervise, and counsel participants, geriatric aides, and family members regarding nursing care.
• Act as nursing consultant to staff and assist medical physicians and nurse practitioners.
• Participate actively in the interdisciplinary team, offering clinical expertise in planning, problem solving, and processes.
• Perform other related duties as assigned.

• Graduate of an accredited college or university with an associate or bachelor’s degree in nursing.
• Current California Registered Nurse license. A California Public Health Nurse certificate is desired.
• Experience in public health nursing is desired.
• Interest, demonstrated ability, and at least one year of experience working with the elderly or frail population required.
• Ability to work as part of a multidisciplinary team.
• CPR certification.

Salary: $54.72 - $72.95 per hour.
