• Date Needed: Ideally by Fall
• Days of the Week Needed: Monday - Thursday 8am - 5pm. Can add in occasional Friday’s as needed
• Experience: General Dentist offering many services and technologies.
• Looking for In-Dentist: Prefer experienced dentist who wants to continue to grow the practice and get involved in the growing Fremont community. We have placed new grads in this location before and would consider again. We have a mentorship program.
• Possible Buy Opportunity: Office purchase can be written into their contract for this location. Office is also for sale outright.

• Patient info: Caseload 20% kids, 80% adults
• Pay types: FFS 32%, PPO 58%, HMO 0%, Medicaid 10%
• Medicaid: 10% not taking any new
• Office info: # of Operatories: 5
