Job Description
Be the vital link between hospitals, rehab centers, and our senior living community! As our Care Transitions Specialist, you’ll help seniors transition smoothly to our care, build strong referral relationships, and make a real impact on residents and their families. This role combines relationship-building, care coordination, and community outreach in a fast‑paced, meaningful environment.
Position SummaryThe Care Transitions Specialist serves as the primary point of contact between hospitals, rehabilitation centers, and our senior living communities. This role focuses on building strong relationships with referral sources, coordinating smooth transitions for residents, and supporting occupancy growth by facilitating timely and efficient admissions.
Key Responsibilities- Develop and maintain relationships with hospital discharge planners, rehab facilities, clinics, and other referral sources.
- Serve as the primary contact for patients and families during the transition to our community.
- Coordinate all aspects of the admission process, including tours, assessments, and necessary documentation.
- Collaborate with internal teams (nursing, social services, marketing, and admissions) to ensure seamless resident transitions.
- Track and report referral sources, occupancy metrics, and admissions activity to management.
- Represent the community at local healthcare and networking events to promote services and build partnerships.
- Provide timely updates and support to families throughout the transition process, ensuring a positive resident experience.
- Assist in marketing and outreach initiatives to increase awareness of community services.
- Experience in healthcare, long-term care, senior living, or hospital discharge planning is preferred.
- Strong interpersonal, communication, and relationship‑building skills.
- Ability to manage multiple priorities, follow processes, and meet deadlines.
- Knowledge of senior care services, LTC/AL/IL communities, and care transitions preferred.
- Proficiency with Microsoft Office and CRM or admissions software is preferred.
- A valid driver’s license and reliable transportation are required for off‑site meetings and hospital visits.
