Position SummaryThe Referral Coordinator strengthens partnerships with external community agencies and supports efficient referral coordination between partners and internal teams. This role focuses on systems-level collaboration, referral tracking, and process improvement to enhance client access across the continuum of care. While this key role may involve limited, referral‑related contact with clients, this team member is not responsible for direct client care, ongoing support, or case management.
Job Duties- Serves as the primary point of contact for community partners, healthcare providers, and other referral sources to support timely and coordinated access to behavioral health services.
- Participate in outreach activities to promote program visibility and strengthen relationships with external stakeholders.
- Develop and maintain an updated and ongoing record of community resources and contacts to facilitate successful referrals.
- Monitor referral progress and maintain regular communication with both internal teams and external partners to ensure smooth transitions into services.
- Engage in limited, referral‑related client contact as needed to support coordination, warm handoffs, and/or referral documentation.
- Document referral activities and client interactions in the electronic health record (EHR) system, prioritizing collaborative documentation.
- Monitor referral outcomes to ensure quality service delivery as well as track and analyze referral trends, identifying barriers, gaps, and opportunities to improve referral pathways and system efficiency.
- Maintain confidentiality and adhere to all ethical standards, policies, and requirements, including HIPAA and client rights and responsibilities.
- Provide support to other staff with assigned duties and responsibilities, as needed.
- Keep leadership informed of challenges, successes, and trends affecting referral processes.
- Perform other job‑related duties as needed.
- Strong interpersonal and communication skills, both verbal and written.
- Ability to build collaborative relationships with community organizations and internal teams.
- Proficiency in Microsoft Office Suite and electronic health record systems.
- Effective problem‑solving and organizational abilities in high‑pressure environments.
- Understanding of trauma‑informed care, person‑centered care, and social drivers of health.
- Proven ability to manage multiple priorities and meet deadlines.
- Awareness and sensitivity of our constituents and the populations served by employees.

- High school diploma or equivalent required; associate or bachelor’s degree in human services or related field preferred.
- 3 years’ experience in human services and/or other behavioral health settings.
- Familiarity with community-based care models and behavioral health principles preferred.
- Experience in community outreach and partnership development with external agencies, healthcare providers, and community organizations.
- Proficient in analytics tools for tracking and improving referral pathways and service delivery.
- Demonstrated understanding of and ability to work with people of diverse backgrounds.
- Work environment will be predominantly in‑office and in the field.
- Prolonged periods of working at a desk and using general office equipment.
- Company‑issued laptop and cell phone.
- Travel to various locations to support program services, as needed.
- Must be able to work a flexible schedule, including evenings and/or weekends (as needed).
- Valid driver’s license, proof of liability auto insurance, and ability to drive a personal vehicle on a daily basis for program needs within the community required.
