Palliative Care Physician Role Overview:
The Palliative Care Physician provides comprehensive palliative care services to veterans, ensuring continuity of care within a multidisciplinary healthcare team. Reporting to the Chief of Medicine, the provider is a key clinical member within the medical center. This role includes a faculty appointment with the University of San Francisco at Fresno, offering the opportunity to engage in academic activities and mentor future healthcare professionals.
The provider will be responsible for the assessment and care of new and returning patients in need of palliative care. Responsibilities include conducting consultations, developing treatment plans, and providing follow-up care both for inpatients and outpatients. The provider will also participate in multidisciplinary team conferences, quality improvement initiatives, and educational activities.
Responsibilities Include:
Veteran-Centered Care:
- Deliver comprehensive palliative care services with an emphasis on symptom management, improving quality of life, patient education, and supporting serious illness navigation.
- Promote shared decision-making with veterans and their families to achieve optimal health outcomes.
Clinical Excellence:
- Perform consultations and develop individualized treatment plans for patients requiring palliative care.
- Manage ongoing care for palliative patients, including symptom control and pain management.
- Conduct outpatient and inpatient palliative care services and provide emergency consultations as needed.
Quality Improvement:
- Participate in continuous quality improvement projects to enhance the delivery of care.
- Utilize various quality measurement tools to monitor and improve clinical performance and patient outcomes.
Clinical Leadership:
- Serve as a mentor and role model for other healthcare providers within the palliative care team.
- Engage in educational activities, providing guidance and support to residents, students, and other trainees.
Healthcare Coordination:
- Coordinate care for veterans involving specialty care, mental health services, and other ancillary services to ensure comprehensive healthcare delivery.
- Collaborate with other healthcare providers to develop individualized treatment plans for veterans.
Use of Technology:
- Utilize the VA's electronic medical record system for documentation, ordering of tests, and communication with patients and healthcare team members.
