** Speech Therapist (SLP) – Full-Time*
* ** Sequoia Transitional Care – Fresno, CA*
* ** Starting at $65/hour (DOE)
**** General Purpose
** The Speech-Language Pathologist (SLP) evaluates and treats patients, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical record. The SLP also conducts in-services and provides ongoing training for facility staff to ensure the highest quality of care and rehabilitation outcomes.
** Essential
Duties & Responsibilities
*** Evaluate patients within twenty-four (24) hours or one business day of physician referral; document any cause for delay in the medical record.
* Develop effective treatment plans and obtain approval for services from the referring physician.
* Provide therapy services according to the physician’s treatment plan.
* Supervise Speech-Language Pathologist Assistants in direct patient care and related activities in accordance with state practice acts.
* Communicate with supervisors, physicians, and interdisciplinary team members regarding patient progress, problems, and care plans.
* Participate in Patient Care Conferences, Utilization Reviews, and Rehabilitation Team meetings.
* Conduct in-service training programs for facility staff as needed.
* Maintain accurate and timely documentation, including evaluations, daily treatment notes, weekly progress reports, recertifications, and discharge summaries.
* Educate patients, families, and nursing staff on maintenance programs and caregiver techniques in preparation for discharge.
* Participate in discharge planning and interdisciplinary care coordination.
* Serve as a clinical preceptor for CFY/RFP Speech-Language Pathologists and student programs.
* Ensure compliance with all applicable state practice acts, federal regulations, and facility policies.
* Exhibit professionalism, empathy, and effective communication with patients, residents, families, and colleagues.
** Supervisory Responsibilities
*** Supervise Clinical Fellows (CFY) and assist nursing in the training of Restorative Aides.
** Qualifications
* ** Master’s or Doctoral degree in Speech-Language Pathology from an accredited program.
* Current, active
** California Speech-Language Pathologist license
** in good standing.
* Proficient in clinical documentation systems such as Casamba and Point Click Care (PCC).
* Must meet continuing education requirements per state practice regulations.
* Strong communication, organizational, and clinical skills.
* Ability to work effectively as part of an interdisciplinary rehabilitation team.
** Physical Requirements
*** Frequent standing, walking, pushing, and pulling.
* Occasional lifting of up to 100 lbs with assistance.
* Occasional stooping, kneeling, and reaching.
* Regular talking, hearing, and close visual attention required.
