The Home Health Physical Therapist is responsible for delivering personalized rehabilitation in patients' homes, conducting assessments, and developing individualized plans of care to improve long-term health and wellness.

The Home Health Physical Therapist is responsible for delivering personalized rehabilitation services to patients in their homes, focusing on improving long-term health and wellness.

This role involves conducting comprehensive assessments, developing individualized treatment plans tailored to each patient’s goals.

• Conduct an initial evaluation/assessment and report findings to the physician/provider and Clinical Manager and develop the plan of care promptly for patients
• Perform physical therapy interventions, procedures, measurements, evaluations and diagnostic tests on patients in the home
• Document initial evaluation and treatment plan and re-evaluations of patient’s progress every thirty (30) days and update the plan of care as necessary
• Complete comprehensive assessments and OASIS data, as indicated on a timely basis
• Assist with orientation, supervision and consultation services to Physical Therapy Assistants
• Observe, record and report to Health Care Provider the patient’s treatment, response and progress and promptly report significant changes and obtain orders to revise plan of care
• Communicate with physician/provider and Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy services
• Provide education and support to patients and their families
• Other duties as assigned

You bring your skills to patients who need it most and deliver exceptional care in rehabilitation and skilled treatment for chronic conditions, illnesses, surgery or injuries to achieve positive outcomes and improve quality of life.

• Master's degree in Physical Therapy
• Active Physical Therapy license in good standing in the state of practice
• At least one year of experience working as a Physical Therapist
• Must have a vehicle, a valid current driver’s license, and automobile insurance
• Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely manner
• Home Health and Home Care Home Base documentation experience

$55.00 - $62.00 per hour
