Bridge Home Health & Hospice seeks a licensed Physical Therapist to join our California home health team. You will provide PT services per the physician’s plan of care and supervise PT assistants as required.
You will assess patients, develop plans of care, educate families, and coordinate with physicians and care teams to improve functional outcomes. A valid CA license and driver’s license are required, CPR certification preferred.
