You will be responsible for providing nursing care to assigned patients under the direct supervision of a registered nurse (RN). General responsibilities may include giving injections and medication, caring for wounds, recording vital signs, tracking medical history, monitoring health status, and keeping detailed documentation on patient progress.

• Provide direct patient home care visits, observe, perform and report preventative, treatment and rehabilitative nursing procedures while implementing the plan of care.
• Observe and report on the home environment and make recommendations to enhance patient care, promote safety and prevent adverse effects.
• Work with patient, family and health care team members to coordinate services, promote quality of care and positive patient care outcomes.
• Take verbal orders from Health Care Providers as necessary within the LVN scope of practice and document appropriately.
• Inform Health Care Providers of changes in patient’s condition in accordance with providers orders and document communications accordingly.
• Provide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventions.
• Provide instructions to patient/CG’s and staff in a timely manner.

• Graduation from an accredited nursing program, current state license as a Licensed Vocational/Practical Nurse and CPR certified.
• At least one-year clinical practical nursing experience. Home Health experience preferred.
• Must have vehicle, valid current driver’s license and automobile insurance.
• Ability to make home visits and provide skilled nursing assessments and care in the home.
