Advanced Practice Clinician (APC)

As an Advanced Practice Clinician (APC) you will be part of a transformative program designed to provide personalized care and support to our patients, including those with complex and chronic needs, all in the comfort of their own homes.

The APC will ensure the delivery of high‑quality clinical and home‑based patient care, while promoting optimal health outcomes and providing cost‑effective solutions that adhere to Alignment Health’s policies, as well as state and federal regulations. You’ll play a vital role in assessing, developing, and coordinating care options tailored to the needs of our members, who are typically 65 and older. With home and virtual visits ranging from 45 to 60 minutes, you’ll provide focused, compassionate care for initial visits, annual check‑ups, and additional support as needed.

    One (1) year of prior clinical or home care experience. Previous EMR experience preferred. Experience in care of older adult (geriatric) patients preferred

• Master’s degree from an accredited NP Program or PA program

  Active California state Nurse Practitioner or Physician Assistant license
• Active RN License and furnishing number (must, upon hire) NP only
• Active Nurse Practitioner Board Certification/Physician Assistant Certification
• NPI Number, DEA, Valid BLS
• Valid California driver license and current automobile insurance

• Yearly Bonus up to 7.5% of annual base salary
