Compassionate Care was established in 2003 in Fresno County and has grown into Central California's most trusted home health agency. We provide in‑home medical, rehabilitative, and support services by seasoned professionals with comprehensive clinical training and robust hospitality experience.

Responsibilities

Patient Care

  • Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
  • Provide physical assessment and review of current and previous illnesses.
  • Utilize all elements of the nursing process as defined in the state Nurse Practice Act.
  • Write and initiate plan of care and re‑evaluate patient status regularly.
  • Participate in revising plan of care as necessary.
  • Use health assessment data to determine nursing diagnosis.
  • Develop care plan with goals based on nursing diagnosis, incorporating palliative nursing actions.
  • Initiate preventive and rehabilitative procedures; administer medications and treatments as prescribed.
  • Provide health care instructions and counsel patients and families.
  • Assist patients with activities of daily living and promote self‑sufficiency and optional comfort care.
  • Act as case manager when assigned, coordinating patient care for caseload.

Communication

  • Maintain accurate and relevant clinical notes regarding patient condition and care provided.
  • Communicate with physician regarding patient needs and report changes in condition; obtain and receive orders.
  • Coordinate care plan with community health professionals.
  • Teach patient and caregiver self‑care techniques and provide medication, diet, and other instructions.
  • Maintain a safe environment for the patient.
  • Assist patient and caregiver and other team members in continuity of care.
  • Work cooperatively with the interdisciplinary group to meet emotional needs.
  • Attend interdisciplinary group meetings.

Additional Duties

  • Participate in on‑call duties as defined by policy.
  • Ensure arrangements for equipment and other necessary items and services are available.
  • Supervise ancillary personnel and delegate responsibilities when required.
  • Maintain and upgrade professional knowledge through continuing education.
  • Fulfill requested or accepted case assignments.
  • Participate in quality assessment and performance improvement activities.
Qualifications
  • Graduate of a National League of Nursing accredited school of nursing.
  • Current California license and CPR certification.
  • Minimum of one year of recent professional nursing experience or a baccalaureate degree in nursing with current RN license; preferred two years of experience in public health, home care or hospice nursing.
  • Licensed driver with insured, operational automobile.
  • Self‑directed, works with minimal supervision.
  • Excellent observation, verbal and written communication, problem‑solving, basic math, and nursing competency skills.
  • Physical ability to walk, stand, lift, and transfer patients and supplies; visual and hearing acuity sufficient for required tasks.
