LPN Med Care Manager

The LPN Medication Nurse provides licensed nursing support in a senior living environment, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities:

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Accurately document in MAR/TAR in real-time
• Monitor symptoms, report adverse reactions or refusals promptly
• Conduct Service & Health Updates (SHUs) and transcribe physician orders
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulations

Your role is critical in preventing medication errors, early detection of health changes, and safeguarding resident safety through precise clinical practices.

Qualifications & Skills:

• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred
