The Oncology & Infusion Nurse Practitioner is responsible for providing advanced clinical care and clinical oversight for patients receiving chemotherapy, immunotherapy, targeted therapies, biologic therapies, supportive medications, and other specialty infusion therapies at the Fresno clinic. The Nurse Practitioner conducts comprehensive patient assessments, evaluates treatment readiness, reviews clinical and laboratory parameters, manages treatment-related adverse events and infusion reactions, and collaborates with physicians, pharmacists, and nursing staff to promote safe, evidence-based care.
The role also supports patient education, accurate documentation, quality improvement, staff clinical guidance, and compliance with organizational policies and applicable regulatory standards.

Scope of SupervisionProvides clinical oversight and guidance to infusion nursing and pharmacy staff. No direct personnel supervision unless assigned by leadership.
Responsibilities of the Oncology & Infusion Nurse Practitioner include the following:- Performs comprehensive initial and ongoing patient assessments, including history, physical examination, medication review, symptom evaluation, and review of relevant clinical information.
- Evaluates patient readiness for treatment and confirms that ordered treatment parameters have been met before therapy begins.
- Reviews laboratory values, diagnostic results, treatment protocols, body surface area calculations, dosing, and provider authorization, as applicable, before chemotherapy administration.
- Provides clinical oversight for the safe administration of chemotherapy, immunotherapy, targeted therapies, biologic therapies, supportive medications, hydration, and other specialty infusion therapies.
- Administers ordered therapies within the Nurse Practitioner scope of practice, approved privileges, provider orders, manufacturer recommendations, evidence-based guidelines, and organizational policies.
- Collaborates with the treating physician and pharmacist regarding treatment plans, dose modifications, supportive care, contraindications, precautions, and changes in patient condition.
- Assesses, identifies, and manages treatment-related adverse events, toxicities, hypersensitivity reactions, anaphylaxis, extravasation, and other infusion-related complications.
- Initiates appropriate interventions during an adverse event, including stopping or pausing treatment, providing emergency or supportive care within scope, notifying the physician and pharmacist, and escalating to emergency services or a higher level of care when indicated.
- Reassesses the patient following clinical intervention and documents the event, interventions performed, notifications made, patient response, and disposition.
- Provides on-site clinical consultation and support to infusion nurses regarding patient assessment, symptom management, treatment parameters, adverse event response, and escalation of care.
- Ensures independent double-check processes are completed for chemotherapy and other high-alert medications in accordance with organizational requirements.
- Promotes compliance with hazardous drug handling requirements, required personal protective equipment, spill response procedures, and applicable USP , OSHA, NIOSH, and organizational standards.
- Assesses and manages venous access devices, including implanted ports, PICCs, central venous catheters, and peripheral IVs, within scope and according to organizational policy and applicable infusion standards.
