Job Description:
This position is responsible for providing skilled nursing care and services to patients in their place of residence, and may case manage nursing and non-skilled care and services when assigned. All nursing services are performed in compliance with state and local laws/regulations, the Nurse Practice Act, and all other healthcare accreditation standards.
What You'll Do:
- Initiate and identify patient needs through assessment
- Provide nursing services based on the plan of care
- Prepare necessary items for treatments and procedures
- Implement and follow current standards to prevent the spread of infection; instruct patients on and, as appropriate, prepare medications for their care and safety
- Make treatment recommendations based on the patient's acuity level and goal potential
- Excel at the admissions process, including same-day electronic documentation
- Remain flexible to accommodate admissions within time, travel, or geographic constraints
- Provide and document skilled care on all visits, meeting agency documentation standards
- Drive throughout the day to visit patients in their homes

Requirements:
- Valid California Registered Nurse license
- A minimum of 2 years of experience in a Home Health or Acute Care nursing setting
- Current CPR certification
- Valid California Driver's License
- Computer skills are a must, including typing skills for reporting and updating documentation in an online portal
- Willingness and ability to work with electronic input and telecommunication devices
- Excellent clinical skills and demonstrated ability to manage time well
- Excellent communication skills; able to communicate clearly with staff and the general public, both verbally and in writing
