The Hospice Registered Nurse Case Manager at At Home Hospice will assume full nursing responsibility for delivering the Plan of Care for each hospice patient, collaborating with a care team to promote physical, psychosocial, emotional, and spiritual well-being in the home setting.
Responsibilities include initial nursing evaluation within 48 hours of referral, developing and coordinating the plan of care, documenting progress, and guiding patients and families.
