We are seeking a reliable and compassionate PRN Physical Therapist to provide one‑on‑one care to patients in their homes. The ideal candidate is self‑motivated, experienced in home health, and eager to help patients reach their full potential.

• Evaluate, develop, and implement individualized treatment plans.
• Deliver direct physical therapy services to patients in their homes.
• Maintain timely and accurate documentation in compliance with Joint Commission standards.
• Coordinate care with the Director of Nursing.
• Manage a flexible part‑time schedule and attend all appointments punctually.
• Supervise and support a growing team of Physical Therapy Assistants (PTAs).

• Degree from an accredited Physical Therapy program.
• Current Texas state licensure as a Physical Therapist.
• At least 1 year of experience as a PT (home health experience preferred).
• Reliable transportation and ability to travel between patient homes.
