Job Summary
Responsible for performing clinical and professional pharmaceutical work in reviewing, processing, transferring and dispensing prescribed medications, with a focus on triaging patients to the appropriate channel of service within the enterprise. In addition, responsible for receiving and making phone calls related to patient care and prescription processing, addressing escalated issues, and handling specialty pharmacy outcomes.
Job Responsibilities- Perform clinical and professional pharmaceutical work including but not limited to:
- Coordinate and complete prescription transfers in compliance with all state and federal regulations.
- Conduct pharmacist functions for specialty pharmacy orders from referral receipt through dispensing.
- Participate in care coordination to meet patient outcomes/goals and assist in resolving complex inquiries and/or complaints escalated by internal/external partners.
- Provide patient counseling in compliance with clinical program protocols and REMS programs; document all interactions with patients and prescribers in the appropriate systems.
- Provide clinical and regulatory oversight to the Logistics Team non‑clinician members handling escalated situations.
- Provide clinical support for escalated patient issues.
- Review patient demographics and prescriptions to ensure appropriate therapy and dose accuracy.
- Update patient records, medication profiles, care plans, initial patient notes, and ongoing progress notes as needed.
- Assist in establishing and refining standard operating procedures to exceed service levels; identify, develop, and implement solutions to resolve problems and remove barriers to performance goals.
- Adhere to established procedures for security of controlled substances, prescriptions, supplies, and quality assurance.
- May support cross‑functional services and high‑touch connections as necessary.
- May support initial patient assessments and medication/administration supplies for optimal outcomes.
- Review work completed by Enterprise Triage Coordinators; provide oversight of Pharmacy Technician duties.
- Other responsibilities as judged or required.

Basic Qualifications- Bachelor's degree in pharmacy or PharmD
- Current pharmacist license in the state of employment
- 3 years of practicing pharmacist experience
- 2 years of experience in customer service, specialty pharmacy, home health care, or related field focused on quality standards
- Strong attention to detail and proven problem‑solving skills
- Willingness to obtain additional state pharmacist licenses as needed
- Experience in a call center, mail service, long‑term care, or specialty pharmacy setting
- Excellent written and verbal communication, leadership, and coaching skills
- Experience utilizing pharmacy and clinical systems
- Focused on customer service, quality, and accuracy
