Care Builders at Home is seeking a compassionate and experienced Registered Nurse (RN) to conduct in-home assessments for clients who require support services in the home. We proudly serve the Collin County area with a team of highly trained and dedicated staff. • Conduct Onboarding in home assessments of clients seeking support services at home
• Create non-medical care plans to include activities of daily living, light housekeeping, personal care, medicationreminders, community transportation, respite care, etc.
• Onboard new clients and periodically reassess client's progress with thein-homecare services
• Educate patients and their families on non-medicalin-homecare services and scheduling
• Utilizeelectronic medical record systems (Axis Care) to accurately document patient information
• Scheduleinitialcaregiver visits

• Pass drug screening
• Valid Driver's License
• Own Reliable Transportation to travel throughout
  
  Prosper and surrounding areas
• Proof of Car Insurance
• Social Security Card or Birth Certificate
• Negative TB skin test
• Provide three work related references
