Community Staff Nurse

We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients.

This post is full time – 12 months fixed term.

Main duties of the job

Our care community team operates an 8 am‑8 pm service based in Princeway Health Centre, covering Frodsham and the surrounding area. You must be able to drive and have access to your own vehicle, with mileage expenses reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidate will undertake duties during working hours that involve the delivery of high‑quality nursing services and holistic, person‑centred care. You will work alongside various professionals to provide seamless care to patients predominantly in their own homes, proactively coordinating their care needs as appropriate and avoiding unnecessary admission to hospital or long‑term care settings.
