The home health nurse provides direct patient care in the patient’s home environment as part of a multidisciplinary team that includes RN, PT, OT, speech, social work and home health techs. The nurse assesses patients and follows individualized care plans that best meet their needs as established by the RN. Direct, one‑on‑one patient care is delivered while communicating any changes in status to the RN Case Manager.
The agency relies heavily on technology; email is the primary means of communication and the nurse uses a laptop to complete documentation in an electronic record. Patients vary widely in acuity, age, and diagnosis. The nurse works under the direction of a clinical team manager and begins with a 6‑10 week orientation with a seasoned preceptor to ensure readiness to treat patients independently.
Additional duties include performance improvement, policy and procedure review, research, and other responsibilities as assigned.

EducationAssociates in Nursing, LPN associate degree, or certificate diploma required.
Certification & Licensure- LPN License required*
- BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required **
- Relevant Specialty Certification preferred
- Must be licensed or eligible to practice pending licensure as a Licensed Practical Nurse in the Commonwealth of Virginia and/or West Virginia (as applicable) with either a multi‑state license, under the Nurse Licensure Compact.
- New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
Valid driver’s license and reliable transportation.
Qualifications- Competent to provide care based on the cognitive, physical, emotional, and chronological stages of human growth and development.
- Competent to provide care using the nursing process.
- Must have a reliable means of phone communication with the hospital.
