Qualifications- Current and valid LPN license in the state of Maryland.
- Graduation from an accredited Practical Nursing program.
- CPR/BLS certification (required).
- Prior experience in a long-term care or skilled nursing facility preferred but not required.
- Strong clinical skills and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Compassionate, patient-focused, and committed to quality care.
In accordance with facility policies and procedures, the LPN provides clinical evaluation of the direct care to residents that is of high quality and under the supervision of the Nurse Manager and DON. Receives input from GNAs and communicates timely with the RN Staff Nurse to meet resident care needs.
Duties and Responsibilities- Provide direct nursing care to residents, including administering medications, treatments, and monitoring vital signs.
- Assist with the development, implementation, and evaluation of individualized care plans.
- Document resident status, treatments, and responses in accordance with facility policy and state/federal regulations.
- Monitor residents for changes in condition and report promptly to the RN or physician.
- Collaborate with interdisciplinary team members, residents, and families to ensure optimal resident outcomes.
- Supervise and support Geriatric Nursing Assistants (GNAs) in the delivery of care.
- Maintain a clean, safe, and therapeutic environment for residents.
- Ensure compliance with infection control, safety, and HIPAA policies.
- Participate in quality assurance activities and staff meetings.
- Fulfills the role of charge nurse when assigned.
- Completes assignments timely, thoroughly, and accurately with sensitivity and caring, seeks assistance when unable to complete work.
- Follows the plan of care as developed by the RN staff nurse, MDS Coordinator(s), or RN members of the Nursing leadership team.
- Observes safety policies and reports any safety concerns.
- Demonstrates respect of resident rights and dignity.
