Job SummaryThe Emergency Room Enhancement (ERE) Care Coordinator plays a vital role in connecting residents to essential services and resources. This position focuses on engaging clients in care, coordinating services, and supporting individuals in achieving and maintaining health and independent functioning.
Key Responsibilities- Engage and support clients who frequently interact with emergency services within the catchment area.
- Provide client assistance in accessing mental health and medical services.
- Assist clients with accessing necessary public services including financial assistance, housing, basic needs for food, shelter, and clothing.
- Aid clients with accessing social, educational, vocational and health and housing resources identified as needs for independent functioning.
- Training, coaching, and supporting daily living skills, including housekeeping, cooking, personal grooming, accessing transportation, developing a budget, paying bills, and maintaining independent residence.
- Provide adequate follow-up with clients regarding appointments, completion and submission of forms or other miscellaneous actions.
- Assist with the coordination of hospital discharges as needed to ensure a smooth transition of care and prevent readmissions or engage existing providers to ensure this care is provided.
- Attend agency and community meetings assigned by supervisor.
- Establish and maintain positive working relationships with local community resources.
- Document all interactions and contacts with clients according to agency and DMH standards.
- Meet all internal and external reporting requirements including data collection for Quality Assurance evaluation and DMH reporting requirements.
- Attend educational seminars and meetings related to professional advancement.
- Assist with daytime mobile crisis response as needed.
- Engage with people in a trauma‑informed and collaborative manner that supports safety, empowerment, voice, and choice.
- Participate in crisis on‑call secondary duties as scheduled.
- Other duties as requested by the supervisor.
- A minimum of a bachelor’s degree in social work, nursing, psychology, mental health, human services, or similar degrees.
- Master’s degree preferred.
- License in a therapeutic discipline or provisional licensing if eligible.
- MUST be able to respond to crises timely, provide CPR and use Crisis Prevention Institute techniques to de‑escalate the situation or other behavioral management techniques.
- Strong interpersonal engagement skills for adults and seniors.
- Ability to develop and maintain strong partnerships with law enforcement, ambulance personnel, crisis call centers, crisis centers and hospitals.
