The overall purpose of the Wellness Nurse position is to provide Person‑Centered Care consistent with accepted standards and expectations of Sunshine Retirement Living and state regulations. The Wellness Nurse’s responsibilities include, but are not limited to, monitoring resident health status, providing physical care, and early diagnosis and intervention of resident changes in health status. Wellness Nurses must communicate politely and effectively with family, primary physicians, and perform all administrative charting or other services as required by regulations and company policy.
In assisted living and memory care, the Wellness Nurse ensures care practices that promote safety, dignity, and engagement for residents with cognitive impairment, while serving as a clinical resource and role model for caregiving staff.
- A current, valid LPN or RN license is required
- Education and/or experience in long‑term care, acute care, or geriatric nursing preferred
- Supervisory experience preferred
- Ability to read, write, and speak English and to verbally communicate with different levels of staff, families, and any outside customers in a professional and polite manner
- No disqualifying criminal or care offenses as defined by regulatory guidelines
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to work under time constraints and meet deadlines
- Maintain a positive and respectful approach with coworkers; ability to work in a team setting and be a team player
- Ability to write nursing/patient progress notes and nursing procedures
- Ability to effectively present information and respond to questions from department heads and customers (patients, family members, physicians, etc.) in a polite and professional manner
- Knowledge of dementia care practices or willingness to complete memory care training
- Satisfactorily pass Sunshine’s and state‑regulated criminal record clearance, health screening, and TB clearance (if applicable)
Job Duties and Responsibilities

- Dedicated to delivering a high level of customer service
- Provides resident care in accordance with accepted standards of practice and within the scope of the LPN or RN license
- Routinely observes and reports on residents’ conditions/changes and then documents in accordance with community policies and as required by regulations
- Administers medication or treatment as prescribed within the LPN or RN scope of practice
- Receives, transcribes, and executes physician orders
- Implements and evaluates resident’s plan of care
- Identifies and secures equipment and supplies; notifies supervisor when supplies are needed
- Ensures appropriate and timely response to resident and/or family concerns
- Responsible for the safety of residents under your supervision
- Communicates resident conditions and expectations to staff at shift start and receives reports throughout and at shift end
- Directs caregivers to assure care is provided per standards of practice and community policies and regulations
- Rounds with caregivers before shift end to assure the unit is in proper order
- Provides guidance and coaching to care staff in managing residents with memory loss, including behavioral interventions, redirection, and validation techniques
- Ensures proper documentation of care, medications, incidents, and changes in condition using electronic health records
