• Perform a comprehensive assessment of psychosocial and medical needs of patients.
• Advocate for patients and assume a leadership role with the interdisciplinary team.
• Coordinate care plans and facilitate patient throughput with a focus on quality and efficiency.
• Act as a liaison through effective and professional communication with physicians, patients, families, hospital staff, and external agencies.
• Perform utilization management reviews and communicate information to third‑party payors.

• Florida RN license or an approved multistate RN license (compact holders have 60 days to obtain the Florida license after relocation).
• BLS‑AHA Provider issued card within 30 days of start.
• Minimum 3 years of RN experience in an acute care setting.
• Prior experience in Case Management preferred.
• BSN preferred.
• Certification in Case Management, Nursing, or Utilization Review preferred.
