Position SummaryTo provide medical care and treatment within an established scope of practice to patients with kidney disease and related conditions. The Advanced Practice Provider (APP) will provide care for patients with acute kidney injury, chronic kidney disease, end‑stage renal disease, and for patients requiring dialysis.
Responsibilities- Inpatient care in the intensive care units, including initial evaluation, management, and discharge planning.
- Provide dialysis services for hemodialysis patients.
- Office care for ongoing treatment of CKD and post‑hospital follow up for end‑stage kidney disease patients.
- Coordinate plan of care, discharge, and specialty referrals as appropriate.
- Perform comprehensive physical examinations and assessments.
- Interpret, analyze, and evaluate patient’s medical record and social data.
- Assist the physician with procedures per collaborative agreement.
- Participate in performance improvement activities.
Monday–Friday, 8:00 am–5:00 pm. Requires weekend coverage (max once per month).
Qualifications- Master’s degree in Nursing with an Acute Care NP certification or graduation from an accredited Physician Assistant program.
- Currently certified as an Advanced Practice Nurse (APN) with Certified Registered Nurse Practitioner (CRNP) Specialty by the Georgia Board of Nursing, or licensed as a Physician Assistant by the Georgia Board of Medical Examiners with certification as a Physician Assistant (PA‑C) by the National Commission on Certification of Physician Assistants.
Gainesville, GA 30501 (relocation required before starting work). The position is in person; no remote work.
