Overview

We are seeking a qualified Nurse Practitioner to provide patient care in the Gainesville area. As a Provider, you will deliver exceptional care in patients' homes and collaborate with a multidisciplinary team.

  • 12‑hour shift (7 days on 7 days off)
  • 8‑hour shift (Monday‑Friday, 8:00 am‑5:00 pm)

Responsibilities
  • Evaluate high‑risk patients and develop individualized care plans.
  • Coordinate care activities with the interdisciplinary team to ensure continuity of care.
  • Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members.
  • Participate in interdisciplinary team meetings to discuss patient progress and care coordination.
  • Foster effective communication and collaboration.
  • Meet staff efficiency expectations while providing high‑quality care in various settings (homes, facilities, telehealth, clinics).
  • Document care management activities accurately for regulatory compliance.
  • Provide patient education and resources related to conditions, treatment plans, self‑management strategies, and preventive care.
  • Offer support and guidance to patients and families.
  • Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
  • Monitor quality measures and gaps in care, identify improvement opportunities, and implement corrective actions.
Qualifications
  • Valid Nurse Practitioner, Physician Assistant, or Physician license in the state of employment in good standing.
  • Experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty care settings.
  • Strong communication, collaboration, and interpersonal skills.
  • Ability to evaluate high‑risk patients, develop care plans, and coordinate activities.
  • Commitment to delivering compassionate, patient‑centered care.
  • Strong organizational and time‑management skills; ability to multitask and prioritize.
  • Basic computer knowledge.
  • Strong written and verbal skills.
  • Leadership skills and ability to foster a cooperative work environment.
  • Ability to work flexible hours, travel between offices/facilities, and maintain a licensed, insured vehicle.
