Overview
We are seeking a qualified Nurse Practitioner to provide patient care in the Gainesville area. As a Provider, you will deliver exceptional care in patients' homes and collaborate with a multidisciplinary team.
Service area(s)- Gainesville
- 12‑hour shift (7 days on 7 days off)
- 8‑hour shift (Monday‑Friday, 8:00 am‑5:00 pm)

- Evaluate high‑risk patients and develop individualized care plans.
- Coordinate care activities with the interdisciplinary team to ensure continuity of care.
- Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members.
- Participate in interdisciplinary team meetings to discuss patient progress and care coordination.
- Foster effective communication and collaboration.
- Meet staff efficiency expectations while providing high‑quality care in various settings (homes, facilities, telehealth, clinics).
- Document care management activities accurately for regulatory compliance.
- Provide patient education and resources related to conditions, treatment plans, self‑management strategies, and preventive care.
- Offer support and guidance to patients and families.
- Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
- Monitor quality measures and gaps in care, identify improvement opportunities, and implement corrective actions.
- Valid Nurse Practitioner, Physician Assistant, or Physician license in the state of employment in good standing.
- Experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty care settings.
- Strong communication, collaboration, and interpersonal skills.
- Ability to evaluate high‑risk patients, develop care plans, and coordinate activities.
- Commitment to delivering compassionate, patient‑centered care.
- Strong organizational and time‑management skills; ability to multitask and prioritize.
- Basic computer knowledge.
- Strong written and verbal skills.
- Leadership skills and ability to foster a cooperative work environment.
- Ability to work flexible hours, travel between offices/facilities, and maintain a licensed, insured vehicle.
