Overview
General Description:
The Case Manager is responsible for providing direct services to tenants including assessment, goal setting and evaluation through formal and informal settings.
As an onsite case manager in a permanent supportive housing program, you are responsible for assisting and supporting tenants in maintaining stable housing.
This entails a variety of duties, including:
- Meeting all residents on assigned case load to ensure monthly goal plans and related documents are completed as prescribed.
- Providing referrals and/or support in maintaining connection to community health services, mental health services, vocational programs, chemical dependence treatment, etc.
- Working with the residents and property management to ensure that the resident fulfills obligations as a tenant.
- Through the combined use of personal vehicles, public transportation and agency vehicles, case managers will actively assist clients with conducting activities in the community.
- Crisis intervention and support.
- Supporting families to ensure their children’s physical, emotional, and educational needs are met and facilitating the children’s participation in developmentally appropriate activities both at Seneca Heights and in the community
- Leading groups as assigned.
- Participating in social activities including celebrations and other on-site events.
- Working collaboratively as a team member to utilize donations, outside resources, and on-site services to meet residential needs.
- Thorough record-keeping, gathering of information for required reports, and updating of HMIS database as well as other administrative duties as requested.
- Other duties, as necessary.
- Licensed mental health clinician (LCSW/LCPC, etc. or LGSW/LGPC, etc.)
- Experience working with vulnerable, at-risk families and mentally ill adults
- Knowledge and commitment to a Trauma-Informed approach
- Excellent oral and written communication skills
- Knowledge of Montgomery County resources preferred
- This position requires a minimum of one evening per week
