Provide case management services and support to individuals with disability and/or medical needs requiring assistance with activities of daily living such as bathing, dressing, toileting, eating and mobility. Responsibilities include establishing positive relationships, creating and monitoring person‑centred plans, ensuring compliance with state mandates, and facilitating access to services.
Responsibilities- Advocate on behalf of clients and educate them about resources and self‑determination.
- Collect and document eligibility information and update plans.
- Engage in person‑centred planning, develop POS, and conduct plan preparations.
- Research, refer, and coordinate resources to achieve client outcomes.
- Monitor plans, conduct follow‑up visits and contacts, and maintain required reporting.
- Adhere to program policies, maintain HIPAA records, and use tracking tools.
- Bachelor’s or Master’s degree in psychology, sociology, social work, nursing, counseling, human services administration, or a related field.
- Valid driver’s license and insured vehicle.
- Experience with persons with disabilities and person‑centered planning preferred.
- Strong problem‑solving, decision‑making, communication and teamwork skills.
Standard office environment with occasional lifting up to 25 lbs. Requires computer usage, driving, and interaction with clients.
