Responsibilities

The Registered Nurse (RN) in Home Health provides and oversees nursing care to patients in their homes, following physician prescriptions and complying with all applicable laws, regulations, and agency policies.

• Coordinate the total plan of care with other healthcare professionals and ensure continuity of care by sharing information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.
• Conduct initial and/or comprehensive nursing evaluations, determine primary care focus, develop the care plan within state guidelines, and submit accurate and timely documentation in collaboration with physicians.
• Perform comprehensive assessments at designated times and develop appropriate care plans.
• Verify patients meet home health eligibility and medical necessity guidelines as defined by the payer source.

Education and Experience

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation (vehicle or public transit)
