• Licensed Practical Nurses (LPN) license
• Registered Nurse (RN) license
• Current CPR certification is required
• Must be able to read, write, speak, and understand the English language.

Our SNF is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team of superstars. Licensed Practical Nurses (LPN) provide direct nursing care to our residents, work alongside a collaborative team, and help ensure that our residents receive the highest quality of care in a safe and supportive environment.

• Assess, plan, and implement individualized care plans for residents.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health status, vital signs, symptoms, and progress.
• Supervise and support a team of CNAs to deliver optimal care.
• Collaborate with physicians, specialists, and other healthcare professionals to coordinate resident care.
• Educate residents and their families on health management, conditions, and treatments.
• Ensure adherence to all state, federal, and facility‑specific regulations and standards.
• Respond to emergencies, assess situations, and provide critical care as needed.
• Participate in quality assurance activities and contribute to the facility’s continuous improvement efforts.
• Weekend Warriors are considered Part‑time (regularly scheduled less than 30 hours per week). In the event a Weekend Warrior CONSISTENTLY works Full‑time hours, they will be considered a full‑time employee and will be removed from the Weekend Warrior Program.
