We’re looking for compassionate, skilled, and dedicated Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide high-quality care to our residents. Job Summary:

As an RN/LPN in our Memory Care Unit, you'll provide skilled nursing care under the direction of the attending physician and within the scope of state nursing practice guidelines. You will play a key role in ensuring each resident receives individualized, compassionate care.

• Monitor and assess residents for changes in condition
• Administer medications and treatments per provider orders
• Provide support with ADLs and personal care
• Oversee and support CNAsMaintain accurate documentation and charting
• Collaborate with interdisciplinary teams to create care plans
• Provide trauma-informed and culturally sensitive care
• Participate in admissions, discharges, and care plan reviews
• Maintain compliance with infection control, safety, and facility policies

• Valid RN or LPN license in the state of Illinois
• CPR certification required
• Compassion, patience, and a strong work ethic
• Ability to work independently and as part of a team
