Pay Range
Base pay: $30.00/hr – $37.00/hr (actual pay based on experience and skills).

Shift Availability
All shifts available for this Memory Care Unit.

Job Summary:

As an RN/LPN in our Memory Care Unit, you will provide skilled nursing care under the direction of the attending physician and within the scope of state nursing practice guidelines. You will play a key role in ensuring each resident receives individualized, compassionate care.

Responsibilities

• Monitor and assess residents for changes in condition
• Administer medications and treatments per provider orders
• Provide support with ADLs and personal care
• Oversee and support CNAs
• Maintain accurate documentation and charting
• Collaborate with interdisciplinary teams to create care plans
• Provide trauma‑informed and culturally sensitive care
• Participate in admissions, discharges, and care plan reviews
• Maintain compliance with infection control, safety, and facility policies

Qualifications

• Valid RN or LPN license in the state of Illinois
• CPR certification required
• Compassion, patience, and a strong work ethic
• Ability to work independently and as part of a team
