Come Join Bridgeway! The System of Care Coordinator provides primary direction and supervision over the System of Care, as the lead agency, as well as in collaboration with the various communities served and partner organizations.
Responsibilities- Work directly with other community organizations and entities to ensure a "no wrong door approach" for individuals needing assistance in mental health, substance use, medical, employment, education, housing, food, etc.
- Act as the primary coordinator of the community system of care, coordinating meetings, communications, intake/services needed by individuals and data tracking for services provided by the overall system of care providers/stakeholders.
- Develop appropriate operational policies, procedures, documentation, etc. for the System of Care in coordination with stakeholders.
- Provide innovative coordinated leadership to assure comprehensive communication, integration of treatment services and coordination with all external resources.
- Educate and train Bridgeway staff as well as external organizations and stakeholders in areas related to the System of Care.
- Ensure all deliverables are met for System of Care funding sources. consumer and stakeholder satisfaction and outcome data with a focus on continuous quality improvement.
Minimum requirements include a Bachelor's degree in Behavioral Health, Social Services or helping profession. Five years' experience in Behavioral Health, Social Services, Healthcare or helping profession is preferred. Experience in coordinating operational systems is also preferred. Employment with Bridgeway is conditional upon completion of all background checks.
