Registered Nurse (RN) – Home Health & Hospice

We’re currently seeking a dedicated Registered Nurse (RN) to join our Home Health & Hospice Department in Galion, Ohio
.
The Home Health and Hospice Registered Nurse provides nursing care to patients as prescribed by the Physician and requested by the patient/caregiver within the framework of the plan of treatment/care in compliance with applicable laws and regulations and the policies of the organization.
Responsibilities- Visit patients in their homes for one-on-one visits
- Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
- Assess patients’ conditions, chart observations, and perform evaluation tasks including vital signs and medication review
- Administer medication as prescribed by the patient’s physician
- Educate patients and families on proper home health care strategies and procedures, and recommend devices or tools that may improve quality of life
- Provide effective communication to patients/clients, families, team members, and other health care professionals
Required:
- Associate’s Degree (A.D.), Diploma, Bachelor’s Degree (B.S.), or Master of Science (M.S.) from an approved nursing school
- Current State of Ohio Registered Nurse licensure
- BLS certification or obtained within 30 days of hire
Preferred:
- One year of nursing experience
- Previous experience in community, health, home health, hospice, or appropriate specialty area
