Position: Social Worker
Job Type: Full Time
NHC Place Sumner is looking for a Social Worker to join our team! The qualified applicant for this position should hold a Bachelor's degree from an accredited program or a related degree.
Experience preferred with a case management background in a SNF / ICF setting, knowledge of MDS and clinical assessments and documentation, care planning, discharge planning, working with support agencies such as hospice, home health and mental health services, etc.
Responsibilities include, but are not limited, to:
- Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
- Working with individual patients and their representatives toward transition back to the community or adjustment to center life and helping navigate crisis of illness and disability
- Serving in an advocacy role for patients and their representatives in expressing and resolving grievances
- Coordinating patient’s plan of care with interdisciplinary team members
- Reporting to the Director of Social Work current case management status and needs
- Maintaining confidentiality of professional information
