Morning Shift LPN

Hickory Grove Post Acute is seeking an experienced and compassionate Evening Shift Licensed Practical Nurse (LPN) to join our dedicated team of healthcare professionals. As an integral member of our nursing staff, you will be responsible for providing high-quality patient care to our residents in a post-acute setting. • Provide nursing care to patients in a post-acute setting, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report changes in patient condition, including vital signs and any medication side effects
• Maintain accurate and up-to-date records of patient care and treatment
• Collaborate with other healthcare professionals to ensure seamless care transitions
• Participate in Quality Improvement initiatives to enhance patient outcomes and satisfaction

• Current licensure as a Licensed Practical Nurse (LPN) in the State of Tennessee
• High school diploma or equivalent required;
  Associate's degree or Bachelor's degree in Nursing preferred
