PRN Physical Therapy Assistant position in Gallup, NM with Continuum Rehab Group. This role involves providing exemplary patient care, building rehab programs to enhance recovery and quality of life, and contributing to a dynamic, clinically driven team.

$40.00/hr - $45.00/hr

Continuum Rehab Group provides in-house therapy with Skilled Nursing facilities in Texas, New Mexico, Indiana, Arizona, and Missouri. This position is PRN/part-time per scheduling needs.

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment interventions to patients with physical functioning disorders under the supervision and direction of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
