Community Action Partnership has an exciting opportunity for Lead Care Coordinator. In this role, the Enhanced Care Management (ECM) Lead Care Coordinator will assist with the development and implementation of program procedures and ensure that requirements are aligned to build sustainable support in impacting the lives of hundreds of residents, playing a critical part in the work and dedication of Community Action Partnership of Orange County's initiatives.

WHAT YOU WILL ACCOMPLISH IN THIS ROLE

The ECM Lead Care Coordinator will deliver intensive, in‑person care management services to the most vulnerable populations, including individuals and families experiencing homelessness, high utilizers, children and youth involved in child welfare, and tenants in CAP OC housing units.
