LPN (Licensed Practical Nurse), Night Shift 7p-7a

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated healthcare team on a night shift schedule. As a valued member of our team, you will have the opportunity to make a meaningful impact in the lives of our residents and their families.

As an LPN, you will be responsible for providing high-quality, patient-centered care to our residents. Your duties will include:

• Providing direct patient care
  , including administering medications, performing vital signs, and assisting with daily living activities
• Observing and reporting changes in residents' conditions to ensure prompt and effective interventions
• Collaborating with healthcare team members
  , including physicians, therapists, and other healthcare professionals, to develop and implement personalized care plans
• Maintaining accurate and detailed records of patient care and treatment

• Deliver exceptional patient care, prioritizing individualized needs and preferences
• Implement care plans in accordance with professional standards and facility policies
• Develop and maintain positive relationships with residents, families, and colleagues
• Participate in regular quality improvement initiatives to enhance patient outcomes and satisfaction
• Maintain a clean, safe, and organized work environment

• Current LPN licensure in the state of Ohio
• High school diploma or equivalent required; diploma or degree in nursing or related field preferred
