Sterling Health Solutions is seeking a highly organized, personable, and dependable Patient Care Coordination & Scheduling Specialist to join our growing Transitional Care Management team. This role works closely alongside our Chief Operating Officer (COO) and plays a critical part in ensuring smooth patient care coordination, efficient provider scheduling, and exceptional communication across patients, clinicians, and internal leadership. Pay Range

$17.00/hr - $21.00/hr

    Richardson, TX (On‑site | Remote Fridays after 90 days)
  • Job Type: Full-Time
  • Pay: $17–$21 per hour, depending on experience
Key Responsibilities
  • Coordinate patient care activities to ensure timely and appropriate services
  • Schedule patient visits with clinicians and healthcare providers
  • Manage and maintain provider calendars, adjusting schedules as needed
  • Serve as a primary point of contact for patients, ensuring a positive and supportive experience
  • Communicate clearly and professionally with patients, providers, and internal teams
  • Support the COO with operational coordination related to patient flow and scheduling
  • Maintain accurate scheduling records and documentation
  • Respond promptly and professionally to patient and provider inquiries
Required Qualifications
  • Strong customer service skills with a patient‑first mindset
  • Excellent verbal and written communication skills
  • Highly organized with strong attention to detail
  • Ability to multitask and prioritize in a fast‑paced environment
  • Comfortable working directly with executive leadership
  • Professional, empathetic, reliable, and team‑oriented
Preferred Qualifications
  • Experience with Transitional Care Management, Home Health, or healthcare operations
  • Experience in healthcare, patient coordination, or scheduling
  • Familiarity with provider scheduling systems or electronic health records (EHRs)
