Klarus Home Health is seeking PRN / Per Diem Licensed Vocational Nurses (LPN / LVN) to provide community-based home health care in the Duncanville, Desoto, Lancaster, TX area.

The primary role of the LPN / LVN is to deliver nursing care under the supervision of a registered nurse to patients of all ages in their homes. The nurse will coordinate with the interdisciplinary team, patients, families, and referral sources to ensure optimal patient outcomes.

Responsibilities:

• Provide nursing care to patients of all ages in their homes under RN supervision.
• Coordinate care with the interdisciplinary team, patients, families, and referral agencies.
• Assist in developing and implementing patient care plans.
• Monitor patient health and report changes to the supervising RN.
• Administer medications and treatments as prescribed.
• Educate patients and families on health needs, conditions, and options.
• Maintain accurate documentation of patient care and progress.

• Graduate from an accredited Vocational/Practical Nursing program.
• Current State license as an LPN/LVN, or multi-state license if applicable, with a valid driver’s license.
• At least two years of clinical experience as an LPN/LVN; home health experience preferred.
• Proficiency in nursing skills and good interpersonal skills.
• Reliable transportation and auto insurance.
• Ability to work flexible hours, travel locally, lift up to 50 lbs, and work in various home environments.
• Capable of making quick decisions under stress to meet patient needs.
