Job SummaryPerform direct patient care and act as support person, advocate and educator within the scope of the Registered Nurse Practice Act, with emphasis on nursing process/outcomes for the Radiation Oncology service line.
Responsibilities- Determine the predictable or unpredictable health status and health needs of patients (individual and family) through interpretation of health data and preventive practice in collaboration with patients and interdisciplinary health care team members.
- Utilize a systematic approach to provide individualized, goal‑directed nursing care.
- Perform comprehensive nursing assessments regarding the health status of the patient.
- Formulate a nursing care plan based on determination of nursing diagnoses and identified problems.
- Implement nursing care within the RN’s scope of practice, including compliance with other laws that apply to the practice setting.
- Develop and implement teaching plans for patients concerning promotion, maintenance and restoration of health, as related to current disease‑specific state.
- Provide education, emotional support and age‑specific care to foster a partnership with the patient and significant others.
- Provide for the care of multiple patients (individual and family) either through direct care or assignment and/or delegation of care to other members of the healthcare team.
- Evaluate the patient’s (individual and family) responses and outcomes to therapeutic interventions.
- Use critical thinking approach to analyze clinical data and current literature as a basis for decision making in nursing practice.
- Provide comprehensive discharge coordination and planning for medically fragile patients with complex discharge needs.
- Coordinate a plan of care, including appointments, transportation, and education, serving as a liaison throughout the facility and in the community regarding services for this unique population.
- Strong interpersonal skills needed to develop and maintain excellent working relationships with physicians, social workers, surgeons, nurses, office staff and other members of the interdisciplinary team.
- Follow patients by phone to continue to navigate patient and help coordinate future medical needs.
- Serve as a supportive point person when patients are readmitted to the hospital.
- Assume responsibility and accountability for the management of resources to achieve efficient, high‑quality outcomes for each patient including support for interdisciplinary collaborations.
- Identify patterns and issues that arise after discharge and implement solutions that improve the quality of care.
- Aid in the education process for new and established referral practitioners and outpatient clinics regarding radiation oncology and Cyberknife. Act as a consultant to multidisciplinary health care team members.
- Other duties as assigned.
- Graduate of an accredited school of Professional Nursing.
- BSN required.
- Previous oncology experience preferred.
- Current RN license in the State of Texas or compact license.
- Current AHA or ARC BLS provider.
- Current AHA or ARC ACLS provider within 6 months.
