Position Summary
The Clinical Pharmacist is responsible for implementation and support of Medicare programs and initiatives across the care management and provider services business lines, as well as multiple value-based contracts. As a member of the multi-disciplinary care team, the Clinical Pharmacist works collaboratively with Care Managers, Health Concierges, Primary Care Providers, and external partners to assure quality medication and disease state management services are delivered to enrolled members.
The Clinical Pharmacist works to support medication adherence efforts, capitalize on pharmacy cost saving opportunities, provide disease state management and education, and provide clinical/ educational support to care teams across the organization.
- Evaluate, monitor, recommend and design patient-specific pharmacotherapeutic regimens.
- Participate in pharmacy activities to assure safe, effective, appropriate, and economical use of medications to improve continuity of care and outcomes.
- Be responsible for managing chronic conditions (diabetes, hypertension, COPD, etc.) and getting these patients to goal.
- Help prevent avoidable medication-related hospitalizations.
- Serve as a resource to practice physicians, nurses, and other team members on the general drug information.
- Regularly monitor patient data and metrics to evaluate program effectiveness and clinical outcomes.
- Understand the medication-based quality measures as used in Pay for Performance, Star, and HEDIS measures.
- Document clinical activities, outcomes, and quality intervention results.
- Provide education on prescribed medications and compounds according to professional standards and practices.
- Participate in committees and meetings to facilitate the improvement of patient care and the development of goals, programs, and objectives for specialized patient populations.
- Participate in problem-solving where appropriate about best practice development for medications such as high cost, or otherwise sub-optimal therapies.
- Adhere to CCNC privacy and security policies to ensure that patient and company data are properly safeguarded.
- Provide effective leadership in staff management with respect to recruitment, selection, retention, training, evaluation, compensation, continued professional and educational development, and communication.
- Assure staff are well trained in and maintain compliance with all organization and department goals, policies, procedures, and business processes.
- Actively establish and promote a positive, diverse, and inclusive working environment that builds trust.
- Assure standards are transparent and applied consistently, impartially, and ethically over time and across all staff members.
- Complete performance evaluations on direct report and set goals for performance and deadlines in line with organization goals and vision.
- Utilize conflict resolution and problem-solving skills to manage issues in a timely manner.
- Mentor and coach staff to facilitate growth for performance improvement.
- Graduate of an accredited School of Pharmacy
- Licensed in the state of NC as a Pharmacist, required
- Doctor of Pharmacy degree and residency, preferred
- 2 years of relevant pharmacy experience preferred
- Ability to gain and maintain access to Hospital/Data or Electronic Medical Record systems
- Critical thinking skills, effective clinical judgment, independent decision making, and problem-solving abilities.
- Computer skills required, including the ability to work with various software applications; MS Office preferred.
- Strong knowledge of organizational Care Management principles, documentation requirements, and performance guarantees.
- Knowledge of and experience with federal and state regulations governing prescription medications and pharmacy operations.
- Excellent communication skills - oral and written.
- Knowledge of Medicare Shared Savings Programs (MSSP), Medicare Advantage (MA), Commercial and Medicaid Plans and the role of pharmacy.
