Overview
The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises nursing assistants to ensure high-quality care in compliance with federal, state, and local regulations. Responsibilities include planning and overseeing daily nursing activities, administering medications, completing required documentation, maintaining patient confidentiality, and performing professional nursing services such as catheterization, wound care, and IV care. The LPN coordinates with physicians, ensures safety protocols, and maintains effective communication with residents, families, and staff to meet residents' needs.
Responsibilities- Be able to perform all functions under your active license as it pertains to your assigned duties.
- Know the residents you serve, providing care based on their care plans and physician orders.
- Prepare and administer medications as ordered by the physician.
- Develop and maintain a good working rapport with inter-departmental personnel, and other departments within the facility to ensure nursing services and activities are adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and shift. Receive/give the nursing report at shift start and end.
Make rounds with physicians as necessary. - Assist in all aspects of a resident’s daily life, including activities of daily living, meal service, and social engagement.
- Appropriately communicate with clinical leadership when a resident’s needs change.
- Know how to care for residents with dementia.
- Be willing to take on other duties and assignments within the scope of your knowledge and experience.

- Licensed Practical Nurse (LPN) with a current unencumbered state license.
- Knowledge of nursing and medical practices and procedures, and awareness of laws and guidelines related to nursing care facilities.
- Experience in Long-Term Care is preferred.
- Willing to take additional training or education as state requirements may require.
- CPR and First Aid certifications are required.
- Ability to be flexible and adaptable while handling multiple priorities.
- Strong written and verbal communication skills.
- Competent in organizational and time management skills; good judgment, problem-solving, and decision-making abilities.

- Required to sit, stand, bend, and walk intermittently throughout the workday; lift and/or move up to 100 pounds.

- May be necessary to assist in the evacuation of residents during emergency situations.
- Visual and auditory ability sufficient for written and verbal communication.
- The noise level in the work environment is usually moderate.
