Join our multidisciplinary primary care treatment team as a Behavioral Health Provider at Kintegra Health.
OverviewWe are looking for a licensed Behavioral Health Provider (LCSW(L), LCMFT(A), or LMFT(A)) to join our family. Under the direct supervision of the Director of Behavioral Health, the candidate will provide behavioral health and substance abuse screening, brief intervention, education, and referral services in our primary care clinics. The role includes ongoing consultation and treatment planning support for primary care medical staff, ensuring holistic, whole‑person healthcare for all patients.

- Minimum two years’ experience in a public health setting
- Master’s Degree
- Licensed Clinical Social Worker (LCSW or LCSW‑A) or equivalent licensure in good standing
- Adhere to professional code of ethics in all practice.
- Provide evidence-based assessment, intervention, consultation, and referral services for patients with behavioral, chronic health, and substance use conditions.
- Work cooperatively within the primary healthcare team to improve patient care and team efficiency.
- Formulate and implement behavioral health, substance abuse, and chronic disease management plans using appropriate modalities and community resources.
- Assist primary care providers with screening, identification, diagnosis, and treatment of behavioral health and substance use issues.
- Engage in ongoing assessment and quality improvement initiatives for primary care/behavioral health integration.
- Provide consultative services and training to healthcare team members.
- Maintain timely documentation of patient treatment, payments, and organizational operations.
- Continuously pursue education to maintain licensure and enhance professional competence.
- Maintain knowledge of and liaison with community agencies.
- Perform other duties as assigned.
