About the Role Monitor and oversee all medical/clinical aspects of services delivered.

Provide medication monitoring and documentation.

Deliver psychopharmacological and medical services within your scope of practice.

Collaborate with ACT team members to monitor clinical status and response to treatment.

Conduct crisis assessments, symptom management, and intervention planning.

Provide individual counseling, substance abuse treatment, and psycho-educational support.

Assist in creating transition plans when clients move in and out of services.

Act as a consultant to primary healthcare providers and community medical staff.

Ensure proper documentation, including treatment plans and progress  available for crisis response and support to the ACT team on a 24/7/365 basis.

Compensation Competitive hourly pay aligned with your role, experience, and contribution, with opportunities for growth over time.

Compensation: $25.02 - $32.13
* Final compensation is determined by factors such as education, licensure level, experience and other qualifications.

Responsibilities

Education/Licensure:
Active LPN license in North Carolina.

Experience:

At least 1 year of experience working with the identified population.
